COVID-19 Vaccine Requirement:

Applicants with medical exams scheduled on or after October 1, 2021, are required to complete the COVID-19 vaccination series before the medical appointment. The administered vaccine should be listed for emergency use by the World Health Organization (WHO) or the US Food and Drug Administration (USFDA) must have licensed or authorized that particular vaccine.

Exemption: Children or young adults who have been advised by National Command Operation Center (NCOC) against receiving the COVID-19 immunization are exempt from this requirement. Fees

Medical examination fees, including x-ray and blood tests, must be paid directly to the examining physician. The basic exam fee is approximately $415 per person.
